Help Required (general questions on pak med school)

Hey guys , i've recently joined this forum .. and i really thought you guys wuld help me out here ...

i am studying in Qatar right now .. and em doing my A level's .. currently em doing my AS level which's year 12 .. and its all from international boards like AQA, EDEXCEL, CIE, and there are more.
I've chosen my sciences to follow the medical path.. - Biology, chemistry, physics.
i am a pakistan and i am willing to study in Pakistan..

I'd like to know how the equivalence certificate is made for O level's and A levels.

It'd also be helpful if you guys culd let me know what the Pakistani medical Universities Require - Grades or Percentages.

Finally i'd like to know if the Pakistani medical Universities require IELTS, SAT or TOFEL on any bases.

It'd be humble if you let me know the information related to my questions

oh yeah guys - there's another part to it..

here wot the thing is that when we do our exams , they are sent off to the UK to be marked under British system thing.

and we were told that the grade boundaries are decided after the exam has been marked

for instance take biology.
one paper of bio here is marked out of 45 in total.
and when the papers have all been marked .. and the highest mark a student gets in the whole board on this particular paper is 23/45.

they allot an A* to the students who get between 18and23/45 an A*.
which's like 40% the minimum u need to get on that paper to get an A* .

so i was curious to know that when we get our equivalence sorted in Pakistan, do they consider their A* on 90% (according to there criteria) .. or do they consider our percentage (40%) on an A*.
