By the time your done with your house job all of this will be behind you and it really won't matter. Once you graduate and get your house job over with, it's a level playing field. Just make sure you are recognized by the PMDC and you are good to go.

The reason a better university is preferred, is that you go through a certain system and exams that trains you for your future. For example, going through the module system is somewhat better because you are more clinically oriented from the start. The syllabus etc also have an impact. It's all just a matter of easing the pain in your future i.e. post graduate exams, house jobs etc require a certain way of thinking and a certain skill set. Some universities confer this better than others. If you go through a college with a hospital that has great clinical exposure you'll be a little more prepared than someone who didn't have as much clinical exposure. It's just a matter of making the process of MBBS the best it can be. But still in the end you'll still be a doctor and eventually will be treated equally.

If it says that the hospitals are attached then yes. But, the hospital closest will almost always be the one that's most used.

The module system is in Shifa, Agha Khan, CMH (and other NUMS colleges) and it may be in Shalamar (they were going to implement it but, I'm not sure if they have yet). I am not aware of any other colleges with the module system.
