You can follow this link to the IBCC website and it tells you what subjects are needed for the conversion:

It seems like you do need Urdu Islamic and Pak Studies.

They also have a document on their website that explains the conversion formula. (Sorry, I am not very familiar with the conversion of O levels grades).

According to the IBCC documentation regarding equivalence for O/A-Levels subjects needed for Pre-Medical, their website states:

Pre-Medical Group: Eight/five ‘O’ level subjects (including English, Mathematics, Physics, Chemistry, and Biology) and three ‘A’ levels (Physics, Chemistry and Biology) with minimum E Grade.

So you need English, Mathematics, Physics, Chemistry and Biology for O-Levels.

Also, in regards to your question regarding the requirements for Urdu, Islamic studies and Pakistan studies, the IBCC states that this requirement depends upon where you studied and appeared for your exams. They state:

"If a Pakistani/dual national candidate studies and appears for examination from abroad, Urdu, Islamiyat and Pakistan Studies will not be considered compulsory requirements."

Hope this helps answer your questions.
